Periodontal Plastic Surgery – Root Coverage

(Isolated Recession Cairo RT1, Lower Central Incisor)

Initial Diagnosis

Reason for Consultation

The patient disliked the appearance and the discrepancy between the gingival margins, and experienced tooth hypersensitivity when exposed to cold and to the touch of the toothbrush.

Initial Diagnosis

Oral Status

Isolated gingival recession (Cairo type 2 recession) on the lower central incisor (#31), with a marked discrepancy in the gingival margin relative to the adjacent teeth, slight interproximal attachment loss, intact papillae, reduced keratinized gingival band, and root exposure with hypersensitivity.

Initial Diagnosis

Why this treatment?

The modified tunnel technique, combined with a subepithelial connective tissue graft harvested from the patient's own palate, is the gold standard for root coverage in gingival recessions. It allows, in a single surgery, the simultaneous resolution of the mucogingival defect, dentinal hypersensitivity, and aesthetic concerns.

Treatment performed

Total duration

Approximately 3 months between the first appointment and the final postoperative check-up.

Number of appointments

4 appointments in total (initial assessment, surgery, suture removal, follow-up).

Treatment phases
  • Initial clinical and photographic assessment
  • Technique planning and selection of the palatal donor site
  • Single root coverage surgery using the tunnel technique, combined with a subepithelial connective tissue graft harvested from the palate
  • Scheduled postoperative follow-up visits.
Materials used

Subepithelial connective tissue harvested from the patient's own palate (autogenous graft). Monofilament sutures 6/0, 7/0 and resorbable sutures.

Teeth treated

Tooth 31 (lower left central incisor).
